Notice to all Patients     

Following a review of our Office Operating Policies, the following Cancellation and No-Show Policies have become necessary are effective immediately. Your anticipated compliance is greatly appreciated:

Cancellation and No-Show Policy

Patient's will be contacted via their preferred method 2 business days (48 hours) prior to their scheduled appointment time. Multiple attempts will not be made to notify the patient. It is the Responsibility of the Patient to acknowledge and confirm their upcoming scheduled appointment via phone or email prior to 24 hours of the scheduled appointment time. If the patient cannot be contacted and a new appointment will only be provided by direct request from the patient and will be based on schedule availability.

If the patient acknowledges their scheduled time and does not show for that appointment, they will be responsible for a $100 no show fee that must be paid before being allowed to reschedule. The patient must also notify the office prior to their scheduled time if they are running late for an appointment. Consideration will be provided to anyone who is running late and contacts the office prior to their scheduled appointment. Anyone who does not contact the office prior to their scheduled appointment and arrives after their scheduled time will be considered a no show and will be responsible for a $100 no show fee. This fee must be paid before being allowed to reschedule based on the availability of the schedule.

Exceptions for late cancellations due to unplanned events and emergencies will be addressed on a case by case basis. Patient may be required to provide some form of documentation upon request.

Typical Hours of Operation are Monday through Friday 8:00am - 5:00pm. If you are having a medical or medication issue do not wait until the end of the day or after hours to contact us for assistance.

Office hours are subject to change including holidays.

Medication Refill Requests

Do not wait until you are about to run out of medication to contact our office. We now require at a minimum two business days to address medication refills. Any requests for medication refills later than 48 hours cannot be guaranteed and unfortunately may result in the patient going without medication. Please do not let yourself run out of needed medication.

 

Please do not rely on the pharmacy to contact our facility for medication refills. All prescriptions provided are written and or called in once verified with the patient thus provided to the specific pharmacy designated by the patient. We will not consider random calls or faxes from pharmacies to fill medications. It is the patients' responsibility to notify us in advance of which pharmacy is being utilized and if a new pharmacy /  location is being considered.
